to help them:

• Build their capacity and ability to design, develop and implement their supports;
• Collaborate with clients to establish person-centered goals and aspirations;
• Understand their plan and what supports are available in their plan;
• Assist with accessing the NDIS portal;
• Link with the most appropriate services to assist them to meet their NDIS goals;
• Coordinate services in their community to best meet their needs and goals with the development of service agreements, service providers and obtain quotes for supports;
• Resolve issues or conflicts which may arise with providers and coach clients through challenges;
• By monitoring their plan outcomes and provide appropriate reports, and prepare for their plan review.
